Passport Services

The Berkeley Public Library now accepts passport applications on behalf of the U.S. Department of State. Passport applications are accepted by appointment only. The library does not accept walk-ins.

Passport Appointment Times (based on availability)

 

To schedule a new passport appointment, please call the library at (708) 544-6017 and let our staff know the purpose of your call. Passport acceptance services are available by appointment only. Following is a list of when the library is open to accept appointments, based on agent availability: 

  • Mondays through Thursdays: 10:00am to 7:00pm

  • Fridays: 10:00am to 5:00pm

  • Saturdays: 11:00am to 4:00pm

Checklist

  • Applicants and relevant parties must be present. 

  • Applicants 16-17 years of age need to appear with one parent (parent's ID is needed) or be able to show parental awareness (You must show at least one legal parent or legal guardian is aware that you are applying for a passport).   • Applicants under the age of 16: Both parents and the child must be present at the time of application. For more information on new passport applications for minors under the age of 16, please visit https://travel.state.gov/content/travel/en/passports/need-passport/under-16.html

  • Applications: Applications may be filled out online at https://travel.state.gov/content/travel.html and then printed single-sided. The Library also provides applications which may be used and must be filled out in black ink. Applications must be completed prior to your appointment. Please do not sign the application until asked to do so by an agent at your appointment. 

  • Valid Photo Identification and Photocopy: Applicants and parents of applicants under the age of 16 must have a valid photo ID, plus a photocopy of both sides.   • Original Proof of Citizenship and Photocopy: Such as certified, original, long form birth certificates, naturalization papers, or a U.S. Passport for applicants over the age of 16, plus a photocopy of both sides. Additional documentation is needed for births abroad as well as both domestic and foreign adoptions.   • Current Passport Photo: A recent 2x2 inch color photo of the full face (eyeglasses removed) with a white background is required. The U.S. Department of State has strict photo guidelines and acceptance of your photo is always at the discretion of the U.S. passport agency. Helpful Tips

  • Have all forms completed prior to coming to your appointment, but please do not sign your application prior to your appointment. 

  • Have all necessary photocopies prior to coming to your appointment. (The Library does offer copying service- $0.10 /page for black and white print; $0.50 for color print). 

  • Allow at least 30 minutes for your appointment. Having an appointment does not guarantee acceptance of your application if the necessary forms are not completed when you arrive for your appointment and/or the supporting identification documents or photo are not provided.

Fees​

Payment of fees - two separate payments are required for the passport fees. Visit https://travel.state.gov/content/travel/en/passports/how-apply/fees.html for more information.

  1. Passport Application Fee: The U.S. Department of State will only accept a check or money order payable to the U.S. Department of State.  

  2. Execution/Acceptance Fee: A separate fee of $35 per application must be paid to the library. The Library will only accept cash or a check or money order made payable to the Berkeley Public Library.

Passport Photo​

The library does not provide photo services. A recent (within past six months) passport photo must be 2 x 2 inches in size with a white background printed on photo-quality paper. The U.S. Department of State has strict photo guidelines and acceptance of your photo is always at the discretion of the U.S. passport agency.
